  • A comprehensive evaluation where history is gathered, the patient and family are assessed, and a treatment plan is developed.

  • We will review current level of functioning, assess growth and challenges, and modify treatment plan as necessary.

    *CPT Codes: 99214 + 90833(typically)

    Fee: $500

  • In depth follow up and more intensive therapeutic interventions targeting specific therapy goals.

    *CPT Codes: 99214 + 90836/90838

    Fee: $750

*Contact your insurance company to determine any Out-of-Network benefits. Many insurance companies will reimburse you for some or most of the cost of treatment once deductibles are met. A superbill will be generated and provided at each visit with the relevant CPT codes listed to be used for insurance reimbursement.
